Summary:Introduction: Ischemic coronary artery disease is a common health problem that requires substantial resources from health care systems. The most common clinical phenotype has been obstructive atherosclerotic disease of the coronary arteries, which has now changed to the phenotype without coronary obstruction, MINOCA. Objective: To present the current status of diagnosis and treatment of myocardial infarction without obstructive coronary artery disease (MINOCA). Method: Information was sought in Pubmed, SciELO, LILACS, LIS and SCOPUS based on the descriptors: myocardial infarction, coronary artery disease, thromboembolism and myocarditis. A database was created with downloaded articles, which after being processed resulted in 26 references. Development: Acute myocardial infarction without obstructive coronary lesions (MINOCA) is a recently defined condition in cardiological practice. Its diagnosis is based on coronary angiography, which initiates an etiological search process that is often complex, but with prognostic and therapeutic importance. From all myocardial infarctions (MI), this one represents 6 to 8%; more frequent in young black women, who suffer Acute Coronary Syndrome without ST-segment elevation. Conclusions: Myocardial infarction without obstructive coronary artery disease (MINOCA), is associated with unconventional risk factors. It should be taken into account that a normal coronary angiogram or an angiogram showing non-obstructive disease does not mean that it is benign.
